How to Participate
If your family has a history of a disease currently being studied at Axial Biotech, we invite you to participate in helping find the genetic cause and help us to improve the treatments available.
- There is no cost to participate and all information is kept strictly confidential.
- You may also be assured that involvement in our research study will not affect the insurability of any family member.
To help advance research simply:
- Let us know that you may be interested.
- Let us know about your family.
- We will contact you and ask you to answer a few questions to determine suitability.
If your family would help advance the research, you will be asked to:
- Sign a consent to participate.
- Complete a short questionnaire

- Provide a release for Axial Biotech to obtain a copy of your medical records and/or x-rays pertaining to the disease being studied.
- Provide a saliva, cheek, or blood sample for DNA.
